GENERAL PURPOSE:
The registered nurse plans, organizes and directs hospice care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
QUALIFICATIONS:
Must hold a valid and current nursing license in the state employed.
Completion of post-secondary education and be able to read, write and follow oral and written directions.
Must be able to relate positively and favorably to patients, families and outside agencies and to work cooperatively with other Employees.
Must be able to maintain regularly scheduled attendance
Nursing degree, with one (1) year of nursing experience preferred.
Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
Must be capable of performing the job function of this position with or without accommodations.
ESSENTIAL JOB FUNCTIONS:
Patient Care
- Provide direct care to patients, under the supervision of the RN Case Manager, in order to maintain the highest level of comfort and quality of life.
- Provide care to patients according to the patient’s specific plan of care as established and modified by the interdisciplinary group.
- Works with all disciplines in meeting patient’s identified needs.
- Communicates with the RN Case Manager and PCC regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required.
- Provide continuity of RN Case Manager’s teaching of patients and family members regarding disease process, medication administration and issues related to patient’s illness.
Documentation Function:
- Make written/oral reports/recommendations concerning the activities of your shift/visit as required.
- Cooperate with other patient services when coordinating nursing services to ensure the patient’s total regiment of care is maintained.
- Participate in agency surveys (inspections) made by authorized government agencies as may be requested.
- Assist in planning the nursing services portion of the patient’s discharge plan as required.
- Admit, transfer and discharge patients, as required
- Completed accident/incident reports as necessary.
- Ensure nursing documentation reflects that the care plan is being followed when administering nursing care or treatment.
Drug Administration Function:
- Prepare and administer medications as ordered by the physician in accordance with federal, state and local regulations and as required by Company policy.
Nursing Care Function:
- Consult with patient’s physicians, as necessary, in providing care, treatment, rehabilitation, etc.
- Administer professional services such as catheterizations, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy and drainage bags, taking blood, etc., as required.
Staff Development Function:
- Attend and participate in all agency in-service programs as required.
- Maintain professional competence through participation in continuing education programs, seminars and training programs.
Safety Function:
- Practice universal precautions at all times
- Adhere to safety rules and regulations
- Ensure self and assigned personnel follow established infection control procedures.
Patient Right’s Function:
- Understand, comply with and promote rules and regulations regarding patient’s rights.
Other Duties:
- Adhere to all of the Company’s policies and procedures.
- Perform other duties as assigned.
